File a Claim

Filing a claim can be complex, but we’re here to help you every step of the way. At Union One, we’ll assign a dedicated Claims Advocate to guide you through the process. While your Claims Advocate doesn’t have the authority to approve or deny your claim, they are committed to assisting you so that your claim is handled efficiently and accurately.

Your Claims Advocate will be your main point of contact. If the insurance carrier’s agents request information or documentation, please send it to your Claims Advocate first. They will forward it on, ensuring a seamless process.

Eligibility & Benefits

You must be an actively working dues-paying Member of the IUEC to be eligible.

If removed from service for reasons other than your medical condition, you will not be eligible.

Your benefits may include:

  • IUEC VIP Plan Benefit: Available after a specified waiting period.
  • Social Security Disability: Depending on your eligibility, benefits may vary.
  • Employer Provided Disability and other Union Provided Disability Benefits: Based on your employer and IUEC Local, you may be eligible for additional benefits that compliment your VIP Plan benefits.
  • Paid Family and Medical Leave (PFML)
  • State Disability Insurance (SDI): States currently known to provide Short-Term State Benefits – CA, HI, NJ, NY, RI and PR.

Please read any provided documents thoroughly, as they contain crucial details about the claims process.

Submit required documents

Provide a copy of last year’s W-2, three months of consecutive pay stubs before your disability date, verification of your last working day, and any relevant medical documents. Instructions on how to submit these will be provided.

Coordinate with your doctor

Share the Medical Qualifications and Physical Demands with your doctor to facilitate determining a return-to-work date.

Important information about this plan

IMPORTANT: The monthly cost for coverage is based on your age at the start of the coverage and will increase on the policy anniversary date after you move into a new age bracket. This benefits guide is tailored for IUEC Members aged 18-70+.

Participation in this program is voluntary, and the decision to enroll rests solely with the Members. Members are responsible for bearing all associated costs. A $2 technology fee is included in all listed monthly costs for the following coverages: Short-Term Disability and Long-Term Disability. A $1 technology fee is included in all listed monthly costs for the following coverages: Member Life and Spouse Life.

IMPORTANT: If you depart from the IUEC, opt-out of paying dues, or retire, you must notify the IUEC Service Center at (224) 487-5030. Not doing so within 90 days could delay or negate your eligibility for a refund. Members have 30 days to notify us of their retirement if they wish to port or convert their Life Insurance.

The IUEC does not contribute to the premiums for this plan on behalf of its Members, does not endorse the plan, and does not require Members to enroll in the plan. Furthermore, the Union receives no financial or other consideration in connection with the administration or promotion of this program.
